    Date you need the items in hand. Please allow at least 2 weeks for completion for standard requests. If your deadline is sooner, please submit your form and follow up with Brittany.

    When will the recipient be receiving this item. This date will be used when factoring in expiration dates. Typical expiration dates are 1 year from the date of the event.

    Please select all as applicable. Please note: Guest Services can only approve General Admission Tickets, Membership Certificates, and Behind-the-Scenes Tours. All other items will need approval from applicable departments which could delay overall approval of requests.
